HSE 160 and HSE 163 Orientation, Fall 2008
Welcome to your first semester on internship! By now, you should have the placement set and approved and you should have worked out a schedule with your onsite supervisor.
In your packet, you will find several important items. On the left side, you should have:
1) The syllabus for the semester. Note that there is one combined syllabus for both HSE 160 and HSE 163.
2) The requirements for the first semester of internship.
3) The internship contract which you should read and sign during this meeting. You will get a copy of this back in the next few weeks.
4) A contractual agreement. You will go over this with your internship supervisor. Parts for you to fill out include the name of your agency, the number of hours per week (9), your name, the semester beginning date (August 25, 2008), and the semester ending date (December 21, 2008. You should review this with your site supervisor within the first week and return it to your instructor by Wednesday, September 3, 2008. You will receive a copy of this for yourself and for your on site supervisor, within two weeks of it being given to your instructor.
On the right side, you should have:
5) Eight double-sided time sheets. These are due every two weeks and it is important that you include your name, which class (HSE 163 or HSE 264), the dates of the week (for example, for your first week, it would be Monday, August 25, 2008 to Sunday, August 31, 2008; weeks start with Monday and end with Sunday), the date you worked, the time when you started the time when you finished, and total hours for each time, then at the bottom, the total hours for the week, the date you completed the time sheet, the signature of your on site supervisor, your signature and the total number of hours for each week. You are strongly advised to make a copy of the sheet before you submit it. The sheet is double sided, and you should have two weeks’ worth of time records, submitted every other Friday.
6) Internship learning plan (2 copies). You have two copies because most likely, it will have to be redone at least once. You can work on this with your onsite supervisor and present it to your instructor by Wednesday of week four. You and the instructor will then review it, and you will have a week or so to revise it, if necessary.
7) Internship evaluation form (3 copies). You will give two of these copies to your supervisor and one copy is for you to keep for your own information. Your supervisor will complete one in October and one in December.
8) Verbatim assignment and two examples.
9) Psychosocial cover sheet and two examples.
